Requirements for obtaining forms

Credit Card Authorization Form

1. Download and print
2. Complete, sign, and date the form
3. Fax the completed form with the following documents to +27 21 851 4955
A photocopy of the FRONT and BACK of your credit card.
A photocopy of your picture identification card.

Travel Booking Form

1. Download and print
2. Complete, sign, and date the form.
3. Send /or fax form back to our Office

China Visa Application Form

2. Complete, sign, and date the form.

The requirements for a single entry visa for the People's Republic of China:

• The visa takes 4 working days (collect on the 5th day). (An extra R250 will be charged for 2 days service and R170 for 3 days) 
• The visa costs R250. (All fees must be paid in CASH or postal orders) 
• One colour photograph with a white background
• One application form
• Copy of airticket or original airticket if first time travelling to China
• Proof of accommodation
• Original letter of proof of employment

For Business/Visitor "F" Visas - these applications are as above but must additionally have:

• An original business letter accompanying the application stating:-
• A formal invitation/authorisation letter or fax from Chinese authorities or institution.
• - nature of business to be done
• - companies to be visited
• - duration of visit

Additional reminders:
Passport (s) must be signed.
Request Visa (s) no sooner then 2 months and no later then 1 month prior to the departure date.

Entry and Exit Vietnam Visa Application Form

A. For South African passport holder

1) Copy of the passport
2) 1 color passport photo
3) R000 cheque made payable to China Horizons for single entry visa (non-refundable)
